Alternatively, we can look up your information using your social security number.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>You will have a brief initial consultation by phone or email to determine suitability, and so that we can be prepared to provide some specific treatment for you at your appointment.<\/li>\n\n\n\n<li>Some visits require only a $3 co-pay per appointment. Some patients have no copay.<\/li>\n<\/ul>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p class=\"has-text-color\" style=\"color:#eccba2\">*Even if your coverage is used up for the year, we usually are still able to provide urgent services.
