- **To be seen as a patient, we must verify your eligibility and coverage prior to your appointment.
- Simply provide us your name, date of birth, and health care number. Alternatively, we can look up your information using your social security number.
- You will have a brief initial consultation by phone or email to determine suitability, and so that we can be prepared to provide some specific treatment for you at your appointment.
- Some visits require only a $3 co-pay per appointment. Some patients have no copay.
